PHYSICALS:
All student-athletes are required to have a physical no earlier than 6 months prior to the first day of practice. All student-athletes not having a physical on file are ineligible and not cleared to participate in preseason or in-season practices or competitions. Physicals are required once during a student-athletes career at Saint Mary's unless severe trauma would require a second physical or release from her or his physician |

SICKLE CELL TRAIT STATUS:
Beginning with the 2014-2015 academic year, the NCAA mandated that all student athletes provide confirmation of their sickle cell trait status. A positive sickle cell solubility test will not prohibit a student-athlete from participation. Sickle cell trait only becomes a threat in certain rare situations in which athletes push the limits of their physical conditioning. Being aware of the trait and taking proper precautions can help trait carriers enjoy successful and healthy athletic careers. Please read the following forms for more information on the sickle cell trait and on how to confirm your sickle cell trait status.

INSURANCE:
As of August 1, 2005 all Division III institutions are required to certify insurance coverage for student-athletes. All student-athletes must verify that they have medical insurance with a minimum coverage limit of $90,000 beginning August 1, 2010, to the University PRIOR to practicing in University sponsored NCAA III athletics. In addition, the coverage limit must be effective during practice and playing seasons and must cover athletically-related injuries. Athletes are to scan their insurance card directly on to the Healthy Roster Website

Please read, fill out, print and sign the following forms. All forms must be returned to the Athletic Training Staff before a student-athlete will be cleared to participate in intercollegiate athletics for Saint Mary's University of Minnesota.

This questionnaire is required by all students prior to their first scheduled practice. The purpose is to gain more information on an athlete’s medical history and to identify any preexisting or predisposing factors that can result in injury or illness |

Emergency contact information for each student-athlete: This must be completed once a year. A copy of this along with the student-athlete’s insurance information will be included in the medical kits |
